    2021 REACH Program Deadlines

    Where A Healthier You is Within REACH
    It’s time to take action and begin working on your REACH goals! Participants can save hundreds on their health insurance premiums, earn additional financial incentives, and improve their overall health and wellbeing. The REACH portal grants you access to a variety of wellness activities including a Health Risk Assessment, courses tailored to your health needs, personalized incentive tracking and more! Get started today!

    The REACH portal not only offers the chance to improve your health, but also helps you earn your 2022 Incentive. Employees and spouses may participate in all three levels.


    Level 1 – Due Aug. 31
    Benefit eligible employee and insured spouse must complete the following:

    • Online Health Risk Assessment
    • Upload Primary Care Well Visit Form with REACH lab measures
      • Note: REACH Labs must be completed at least 24 hours prior to your PCP wellness visit.

    Health Insurance Premium Credit

    • $780 Employee
    • $520 Spouse*

    *Employee must complete Level 1 for spouse to be eligible for spousal credit. All credits are applied to the employee health plan premium.

    Employee Financial Incentive $100

     


     

    Level 2 – Due Dec. 15
    Benefit eligible employee and insured spouse must complete the following:

    • Meet 2 out of 3 Key Health Targets
           OR
    • Make a 5% improvement from 2020 in Key Target
           OR
    • Complete an Alternative Standard for the missed Key Target

    2021 Key Health Targets

    1. BP ≤ 140/90
    2. A1C < 5.7
    3. BMI ≤ 25 or Men’s Waist less than 40″ – Women less than 35″

    Additional Employee Financial Incentive: $300

    Additional Spouse Incentive: Raffle entry for a chance to win an Apple Watch (Winner will be chosen in Dec. 2021)


     

    Level 3 – Due Dec. 15
    Benefit eligible employee and insured spouse must complete the following:

    • Self-report 120 Healthy Habit Points in 2021
    • Mammogram is required as part of these points for women over the age of 40

    Additional Employee Financial Incentive: $120

    Additional Spouse Incentive: Raffle entry for a chance to win a $50 gift card (Winner will be chosen in Dec. 2021)

    Total REACH Wellbeing Incentives Available

    Employee and Spouse: Health Premium Credit $1300 / Financial Incentive $520
    Employee: Health Premium Credit $780 / Financial Incentive $520

    REACH Portal Enhancements

    The REACH portal will be down from January 1 through January 18th for platform enhancements.

    Please log in on January 18th to see the newly updated REACH Platform! Healthy Habits For COVID Vaccination- Get a Jump on your Healthy Habits for 2021. Earn 60 Points! 

    Complete both steps of  your COVID vaccination and earn 60 points towards your 2021 REACH Level 3 Healthy Habits! This accounts for half of your total points required in Level 3. Points will be updated automatically if you receive both doses of the vaccine at Riverside. No need to self-record!

    If you received the COVID-19 Vaccine outside of Riverside, you must provide documentation to the REACH Department of the second vaccination for the points to be awarded.

    Crain’s Chicago Business announces award
