2022 REACH Program

The REACH portal not only offers the chance to improve your health, but also helps you earn your 2023 Incentive. Employees and spouses may participate in all three levels.


Level 1 – Due Aug. 31
Benefit eligible employee and insured spouse must complete the following:

  • 2022 Participation Consent Form
  • Online Health Risk Assessment
  • Upload Primary Care Well Visit Form with REACH lab measures
    • Note: REACH Labs must be completed at least 24 hours prior to your PCP wellness visit.

Health Insurance Premium Credit

  • $780 Employee
  • $520 Spouse*

*Employee must complete Level 1 for a spouse to be eligible for spousal credit. All credits are applied to the employee health plan premium.

Employee Financial Incentive $100


Level 2 – Due Dec. 15
Benefit eligible employee and insured spouse must complete the following:

  • Meet 2 out of 3 Key Health Targets
         OR
  • Make a 5% improvement from 2021 in Key Target
         OR
  • Complete an Alternative Standard for the missed Key Target

2022 Key Health Targets

  1. BP ≤ 140/90
  2. A1C < 5.7
  3. BMI ≤25 or Men’s Waist less than 40” – Women less than 35”

Additional Employee Financial Incentive: $300

Additional Spouse Incentive: Raffle entry for a chance to win an Apple Watch

(Winner will be chosen in Dec. 2022)


Level 3 – Due Dec. 15
Benefit eligible employee and insured spouse must complete the following:

  • Self-report 120 Healthy Habit Points in 2022
  • Mammogram is required as part of these points for women 40 years and older

Additional Employee Financial Incentive: $120

Additional Spouse Incentive: Raffle entry for a chance to win a $50 gift card (Winner will be chosen in Dec. 2022)

Total REACH Wellbeing Incentives Available

Employee and Spouse: Health Premium Credit $1300 / Financial Incentive $520
Employee: Health Premium Credit $780 / Financial Incentive $520

Employees who have set up an account in 2021 can use their existing credentials to log in or use single sign-on (SSO) by clicking on the REACH icon found on Rivernet or the Employee Self Service Portal.

Insured spouses may create their REACH Account at assethealth.com/REACH using the following login information. Employees can also use this URL to create a mobile app shortcut on their smartphone.

Username: first seven letters of last name plus first initial (like your City email)
Password: first initial and last initial plus last four digits of SSN

First-time users: You will be asked to change your password after logging in.

Download the Asset Health Mobile App to easily access the REACH wellness portal from your mobile device. The app is available for download in the Apple App Store and Google Play stores.

The REACH Portal will be closed from December 30th thru January 3rd to transition into the 2022 REACH Portal. The REACH Wellness Credits will be on the first full check in the New Year on 1/28/2022.

Retrain Your Brain: Week 3

Welcome the third week of the Retrain Your Brain Challenge!

Featured Challenge: Mindfulness Videos
Log into the REACH portal to watch one of five short relaxation videos with calming visuals and different guided lessons on mindfulness. Don’t forget to check out the other four activities available this week, too!

Your Occipital Lobes

The mindfulness videos this week engage your occipital lobes that:

  • Manage your visual interpretation

Stress Tip: Take a few moments throughout the day to practice mindfulness.
Practicing mindfulness hones our attention for detail and encourages us to stay focused and efficient, which reduces the stress we encounter each day.

Before you even get out of bed:

  • Take a few deep breaths to allow your body to transition from slumber to wakefulness and set the tone with a relaxed approach to your day.
  • Determine a small goal or intention for the day.

In the afternoon:

  • Pause for a moment or two to notice how you’re feeling and see if you can adjust things like your posture or grab a drink of water away from your workspace to give your eyes a rest.

Before you go to sleep:

  • Take a second to stretch out any muscles that might be tight and try to let go of your day. After all, tomorrow is a fresh start.

Log in at www.RiversideHealthcare.org/REACH. Select the different sections of the brain to complete the daily activities. Earn one point per activity, for a maximum of five points each day.

Collect 70 points to complete the challenge before Oct. 31.
