DAISY Winner: Daniella
We are celebrating Cardiac Rehab RN Daniella as our most recent DAISY Award Winner. The DAISY Award is an honor that recognizes the exceptional, skillful, and compassionate care these nurses demonstrate for our patients and families every day. #DAISYAward
Her nomination read: “Daniella had a patient come in for a regular appointment on Monday. Upon arrival the patient informed the team that she was currently homeless. This patient has been struggling with limited resources since her surgical procedure and was not yet cleared for return to work, so has had no income since surgery. Daniella took the initiative to make sure the patient had resources in place prior to leaving the cardiac rehab gym. The patient had not had any food that day, so the staff were able to get her nourishment while at her appointment as well as provide snacks she could easily carry, until she could access the food pantry. Daniella took the extra steps to contact Kankakee county health department, Aunt Martha’s, and Riverside population health department so she could connect with a social worker. With the help of population health the patient was able to find alternative living arrangements, a referral to Riverside’s food pantry and early follow up with her provider to get cleared to return to work. Daniella’s quick action showed the kindest touch when this patient really needed it and ensured the patient would continue to receive the care needed and continue to heal. The goal of cardiac rehab for most patients is a return to independence and improved overall health. Daniella works very hard to provide excellent care to all her patients.”
